Park Facility Request Form

All requests are processed on a first-come, first-served basis. Park facilities may be booked 12 months in advance. You may obtain a Facility Request Packet from the Herculaneum City Hall or use the form on this page.

a. Fill out the form completely, including the date and time for the request, as well as your contact information and return it to the Herculaneum City Hall or email to info@cityofherculaneum.org . The request must have the signature of the person officially making the request and who will responsible for all actions of participants at the function. This person is required to be present during the entire function.

b. Non-profit Civic clubs and organizations with a 501(c)(3) status may request to use the facilities free of charge by attaching a letter of waiver from the organization to this application.

Facility Reservation Rules/Policy

  1. Prohibited Use for Political Activities: Park rentals may not be used for political purposes, including but not limited to fundraising, campaigning, debates, or similar events.
  2. Banners and Advertisements: Without prior approval, posting banners, advertisements, or signage on City property is strictly prohibited.
  3. Alterations to Property: Digging holes, installing temporary or permanent fixtures, or any other modifications to City property are not allowed.
  4. Food Distribution or Sales: Public events that involve selling or distributing food require an appropriate health permit in accordance with local regulations.
  5. Prohibition of Glass Containers: For the safety of all patrons, glass containers are not allowed in the park.
  6. Damage and Cleaning Responsibility: The group reserving the facility will be held financially responsible for any property damage or failure to leave the facility clean. Associated repair or cleanup costs will be charged to the responsible party.
  7. Chaperone Requirements for Youth Events: Responsible adults must supervise youth events. A minimum of two adult chaperones is required for the first 40 youth, with an additional chaperone required for every 20 additional youth.
  8. Insurance Requirements: Depending on the nature of the event, liability insurance coverage naming the City as an additional insured may be required.
  9. Liability Waiver: The Parks and Recreation Department is not liable for charges incurred by the reserving group or for any injuries sustained by participants or individuals hired for the event.
  10. Onsite Event Supervision: The individual reserving the facility must be present throughout the event and authorized to make decisions on behalf of the group or organization.
  11. Communication of Rules: The reserving party is responsible for ensuring all attendees are informed of the Park and Facility Rules and Regulations. Failure to comply may result in immediate loss of privileges or denial of future rental requests.
  12. Noise Levels and Disturbance: Events must comply with local noise ordinances to avoid disturbances to surrounding areas. Per Ordinance No. 15-2009 §§1, excessive noise or disruption is prohibited, and violators may face penalties, including the loss of future rental privileges.
  13. Tobacco-Free Policy: To reduce the negative impact of tobacco use on parks and recreation patrons, the use of all tobacco products, including e-cigarettes, is prohibited in any park except in designated areas.
  14. Pet Restraint and Cleanup: Per Ordinance No. 121-2006 §1:
    • Pets must be on a leash not exceeding six (6) feet in length while in public parks.
    • Pet owners are responsible for cleaning up any waste their animals leave.
  15. Prohibited Items: Water balloons, confetti, or similar items that may cause litter or damage are strictly prohibited.
  16. Event Timelines: Events must conclude by the designated end time specified in the rental agreement. Failure to vacate on time may result in additional fees.
  17. Parking Regulations: Attendees must park in designated areas. Blocking fire lanes, driveways, or unauthorized areas is not permitted.
  18. Permit Display: All required permits must be available for inspection during the event.
